Joint and Soft Tissue Injection

Even if a quality service was available in secondary care, in a time scale that meant our patients were treated in a few weeks rather then months, there is still a place for primary care treatment of joints and soft tissues. The problem is that many of us in general practice have either never been trained in injection therapy or have forgotten the skills required. In these days of stressed secondary health services, it becomes incumbent on more of us to acquire those skills.

Several tomes exist on the subject; the beauty of Trevor Silver’s book is that he takes a structured and simple approach. He starts with a short introduction and gives a synopsis of the incidence of rheumatic disease and of the type of conditions amenable to injection therapy. He deals succinctly with choices of steroids and local anaesthetic, contraindications, how often to inject and post-injection advice. He then deals with the medico-legal issues in just four pages.

The body of the book then takes us through the various parts of the body that are amenable to injection in the primary care setting. From the shoulder to the foot each area gets its own chapter. The format of the chapters is constant in style, making for easy reading and appreciation. Common problems are presented and then each joint or trigger point is illustrated with excellent and easy to follow colour photographs and colour diagrams. The diagrams are simple "under the skin" anatomical drawings of the accompanying photograph. This makes appreciation of the anatomical landmarks simple for the operator.

The book finishes with a chapter on imaging and other therapeutic options. The X-rays and ultrasound pictures are well labelled and easy to understand.

All in all, I can thoroughly recommend this book with no greater praise than it is the first choice for my practice.
